Certified Home Health Aide (CHHA)
Personal Care Assistant (PCA) Services

All care is set up, supervised, directed and reviewed by our registered nurse supervisors.

Personal Care Needs:
Hands on assistance with bathing, showering, grooming (shampoo, shave, oral hygiene, skin care, lotion/massage) and dressing activities.

Activity Needs:
Turn/position patient, dangle, assist/perform transfers, hands on assistance with ambulation (with cane,
walker, crutches or wheelchair activities) and prescribed range of motion exercises. Assist in making and
providing transportation to appointments and outings.

Treatment Needs:
Recorded vital signs (temperature, pulse, blood pressure, respiration) ice bag/heat applications,
intake and output of food and fluids.

Elimination Needs:
Assistance with toileting and use of bed-side commode or bedpan, catheter care, empty/change
catheter/colostomy drainage bag, check daily bowel movement, fecal/urine collection for testing.

Household Needs:
Meal preparation, linen change, laundry, light housekeeping, (kitchen, wash dishes, bedroom, bathroom),
and grocery shopping.

Safety Needs:
Medication reminders, observation and reporting of change in client’s mental/behavioral/physical status to
our registered nurse supervisors, maintaining safety precautions (remove trip items, use side
rails/transfer belt/support bags as needed).

Skilled Nursing Care/Therapy Services
Licensed Practical Nurse (LPN) and Registered Nurse (RN) Care

All care provided is directed under a physicians orders and both supervised and set up by our registered
nurse supervisors. Skilled services include but are not limited to:
Medication administration / teaching & wound management / Ostomy/Tracheotomy care / IV therapy /
Urological care / Diabetic teaching & care / care will also encompass all HHA/PCA duties.

Note: Licensed nurses cannot perform all duties that registered nurses can
such as assessments supervision and administering blood products.

Therapy Physical (PT)/Speech (ST)/Occupational (OT)/ Massage (MT)
In home rehabilitation focusing on muscle and joint strengthening during home visits with prescribed home
exercise plan between appointments and upon discharge.

In Home Rehabilitation

• Spinal cord injury and head trauma rehabilitation • Cardiac/stroke care • Post and pre-operative care • Orthopedic/joint replacement surgery

Respiratory Therapy

• Ventilator certified nursing care • Aerosolized drug administration • All respiratory services and oxygen therapies
